11 weeks old, died unexpectedly on November 1, 2015 at St. Rita's Medical Center surrounded by his family.

Chad was born on August 7, 2015, in Lima to Chad Clemons III and Kimberly Davis whom survive in Lima.

He is survived by, grandparents, Pepper Richardson & Eric Foltz, Brenda Tenwalde & Dannie Davis III, Desiree & Shane Gilman, Chad Clemons Jr. and the late Tonya; great grandparents, Violet & Dannie Davis Jr., Sandra & Ray Huster, great great grandmother, Edna Herring, great grandfather-Jack Long; aunts, Jessica & Monica, uncles, Dannie, Deklen, Randy, Koen, Corey and Hunter, God parents, Byron Kohli & Merissa Kimble.

The family will receive friends from 11 a.m. to 1 p.m. on Friday November 6, 2015 at Chamberlain-Huckeriede Funeral Home in Lima, Ohio.

A memorial service will begin at 1 p.m. on Friday, November 6, 2015 at Chamberlain-Huckeriede Funeral Home.
