86, died at 12:21 AM on Monday, December 01, 2014, at Lima Memorial Health System in Lima, Ohio.

Phyllis was born on December 16, 1927, in Wapakoneta, Ohio, to the late Herbert M. Wentz and Grace M. (Cooke) Wentz.

She was a member of Trinity United Methodist Church; Chairman of its Music Committee; member & Past President of Chancel Choir which she sang in for over 50 years; also past president of the choir and served on the Worship Commission as its first chairman and also served as Chair. of Music Committee 3 different terms; Charter member of the Board of Trustees of the Friends of Music and a member of the Northwest Civic Music Board; a member of the Trinity Chapter, #16, Order of the Eastern Star, served as worthy matron in 1958 and 2006. She was a member of the Past Matron's Club; a grand page in the Grand Chapter, OES in 1968 and served as the president of the grand pages and also served on the Ohio Grand Chapter, OES Heart Committee in 1988; During the 40's&50's she worked as a copy girl & reporter for the Lima News and reporter and associate women's editor at the Lima Citizen where she wrote a weekly comumn called "Music To My Ears"; Served on the YWCA Board of directors and emceed the first Rose Queen Gala; member of the advisory board of the Lima Assembly No 78, Order of Rainbow Girls; She joined the American Heart Assoc. in 1964 serving for 31 years starting as Field Consultant and the Community Manager. When she started she covered 11 counties in West Central Ohio and during her years with the Heart Assoc. she raised more than $5.7 million dollars for research and community education. In 1994 she was named the Ohio Affiliate community Manager of the Year and in 1995 the national Executive vice president presented her with the Time, Feeling and Focus Award.

She is survived by: several nieces and nephews, Eric S. (Beverly) Wentz, Shari (Daniel) Meconis, Jeffrey S. Gehrlich, Pamela (Mike) Harman, Corliss (Steven) Bair, , Christopher (Sue) Gehrlich; several great nieces and nephews, Brett (Nina) Meconis, Stephanie(Steve) Lee, Matthew Harman, Andrew Harman, Erika Wentz (Doug) Russell, Jason Conley, Brandon (Heather) Bair, Austin Gehrlich, Chase Gehrlich; great great nephew, Mason Meconis; and dear friend & caregiver, Ruth Boyd.

She was preceded in death by: three sisters, Jean Anne Wentz, Kathryn Cain and Constance Gehrlich; a brother, F. C. "Bud" Wentz.

The family will receive friends from 2:00 PM to 4:00 PM and from 6:00 PM to 8:00 PM on Thursday, December 04, 2014 at Chamberlain-Huckeriede Funeral Home in Lima, Ohio.

A funeral service will begin at 10:00 AM on Friday, December 05, 2014 at Trinity United Methodist Church. Rev. David Harris will officiate.

Interment will be in Greenlawn Cemetery, Wapakoneta.
